On September 11, 2001, our lives changed forever. In an effort to protect you and our country, the USA PATRIOT Act was signed into law. To help the government fight the funding of terrorism and money laundering activities, this Federal law requires all financial institutions to:
Verify the identity of any person seeking to open an account (which includes deposits, loans, safe deposit boxes, retail investments, and trust accounts); and
Maintain records of the information used to verify the persons identity.
We proudly support all efforts to protect and maintain the security of our customers and our country. Therefore, when you open an account at United Community Bank we will ask you for your name, address, date of birth, and taxpayer identification number. We may also ask you for picture identification, such as your drivers license, so that we can verify the information that you have provided us. In some cases we may need to verify your information through independent sources such as a consumer reporting agency, employer, or other banking reference.
This application is designed to be completed by the applicant(s) with the lender's assistance. Applicants should complete this form as "Borrower" or "Co-Borrower", as applicable. Co-Borrower information must also be provided (and the appropriate box checked) when:
the income or assets of a person other than the "Borrower" (including the Borrower's spouse) will be used as a basis for loan qualification
the income or assets of the Borrower's spouse will not be used as a basis for loan qualification, but his or her liabilities must be considered because the Borrower resides in a community property state, the security property is located in a community property state, or the Borrower is relying on other property located in a community property state as a basis for repayment of the loan.
